2. If you decide we are a good fit, complete the New Patient Screening Form at the end of the course. You will then be contacted via email within 48 business hours.
3. Next, complete the questionnaires we send you and share your history before your 60-minute Initial Visit Appointment (must complete at least 1 week before)
4. Attend your Initial 60-Minute Appointment with Kate Willis, NP-C or Dr. Polly Watson
5. If you want to continue with care after the first appointment, you’re invited to join HWMD Membership. (scroll down for pricing and details)
6. Educate & Engage: Members enrich their health by learning from the 100+ educational video archive and participating in the members-only Facebook Group.
