Preparing for your first visit

Welcome, we are so glad you are here. Taking this step is a big deal, and we want your first visit to feel like time well spent. The more your provider knows about you before you walk in, the more of that hour goes to you. Here is what we will need and the easiest ways to get it to us.

Please send your questionnaires at least one week before your visit. They are best completed on a computer using Google Chrome rather than a phone or tablet. If we do not have enough time to review your case, we may ask to reschedule.

Preparing for your visit

Help us make the most of your appointment by taking a few moments to prepare:

  • Complete your pre-visit questionnaire. This helps your provider understand your current concerns and priorities so your appointment time can be spent on what matters most to you.
  • Update us on any changes. Let us know about any new medications or supplements prescribed or recommended by other providers, as well as any changes in your health, symptoms, or wellness goals since your last visit.
  • Review your recent testing. We can access records from Labcorp, Quest, Raleigh Radiology, Wake Radiology, and Cardinal Points Imaging before your appointment, so there is no need to send us those reports. If you have completed testing elsewhere, please upload those results to your Charm Patient Portal using the instructions below at least 48 business hours ahead of your appointment.
How to share your records
  • Email PDFs to support@hormonewellnessmd.com, which is HIPAA compliant. Put the date of your appointment and your provider’s name in the subject line, for example “Medical records for [date/time] with [provider name]”.
  • If you need us to request them: complete a Release of Information form and send it to us through Charm or by email.
What to bring, and whether to bring someone

Bring any supplements you are taking in their original containers. If we can use something you already have, we will.

If it helps to have a partner or friend along as another set of ears, they are welcome to be present for your visit. If you would like privacy during part of your consultation or during your exam, let us know and we will show them to the waiting area.

What if I cannot finish everything in time?

You will get more out of your appointment if we have reviewed your intake forms in advance.

  • If you are scheduled with Kate Willis, FNP, IFMCP, MSCP, all questionnaires must be complete at least 48 business hours before your appointment, or we will ask you to reschedule.
  • If you are scheduled with Dr. Watson, we will use some of your appointment time to gather this information, and you may need additional follow-up appointments.
